Aims
Enable persons living with dementia (PLWDs) to remain in the community and support caregivers in managing safety and behavioral concerns.
Background
SLH expanded from inpatient dementia care to a multidisciplinary home-based service for PLWDs and caregivers. The project aims to reduce hospital admissions and improve community support.
Methods
Provided assessments, interventions, care coordination, service linkages, and caregiver education. Used the Humanitude approach and CWM Toolkit for caregiver training.
Results
Served 636 PLWDs and 611 caregivers, reducing hospital admissions. Caregiver burden improved based on Zarit Burden Interview (ZBI) scores, and satisfaction rates were high.
Conclusion
The project successfully enhanced dementia care in the community. Future plans include expanding the Humanitude training program and strengthening collaboration with community partners.
Lessons Learnt
Increasing engagement and coaching for caregivers reduced behavioral issues in PLWDs. Training and upskilling healthcare professionals are essential.
