Frailty imposes a burden on healthcare, but its effects are reversible through holistic interventions.
Methods
Partnered SACs for health coaching and social prescribing.
Results
Significant improvements in health indicators: Systolic BP (-6.57mmHg), Nutritional (+0.4), FRAIL scores improved in 46% of participants.
Conclusion
Scalable to other regions, creating a standardized national care model.
Lessons Learnt
Digital solutions and flexibility are crucial for scaling and sustaining outcomes.
