Baseline: 25% ACP uptake and 15% die-in-place rate; gaps in staff training and resource availability.
Methods
Adopted a 3-pronged approach involving training, system reviews, and specialist support.
Results
ACP rate improved to 95%; 63% of residents died in place vs. 15% baseline; cost savings of up to $522,238.
Conclusion
Breaking processes into manageable steps significantly improves care quality.
Lessons Learnt
Understanding resident preferences and team-based care improves EOL outcomes.
