Aims
To improve the percentage of patients (discharged home) with a length of stay (LOS) ≤ 26 days from 53% to ≥ 64% by 31 Mar 2023.
Background
Hip fracture is prevalent among older adults in Singapore, with extended LOS impacting care quality and resource efficiency.
Methods
Adopted a multidisciplinary team approach using the Plan-Do-Check-Act cycle and conducted a 3-month pilot in rehabilitation wards to refine workflows before full implementation.
Results
Increased percentage of patients discharged home with LOS ≤ 26 days to 64%; improved staff satisfaction and reduced unnecessary rework between SACH and CGH.
Conclusion
Enhanced care processes reduced LOS, improved staff satisfaction, and set the stage for integrating technology like robotic rehab for future efficiency.
Lessons Learnt
Multidisciplinary collaboration and data analytics are critical for sustaining improved care processes and outcomes.
