Aims
To ensure 100% of identified serious reportable events (SRE), clinical, and medico-legal (ML) cases are accessible by staff across JurongHealth Campus by April 2019.
Background
Staff were unaware of SRE and ML cases, inhibiting effective clinical governance and organizational learning.
Methods
Created a learning repository, mapped processes to identify information gaps, and implemented iterative PDSA cycles for feedback.
Results
Achieved target with improved knowledge sharing and increased case views from 18 to over 33 per month.
Conclusion
The project established a culture of learning and accountability, promoting safer healthcare practices.
Lessons Learnt
Knowledge management is crucial for clinical governance, risk management, and continuous improvement.
Additional Information
Recognized as an effective quality improvement initiative; the repository is being expanded for more user-friendly access.
