Background
Diabetes affects a large number of inpatients, with high complication rates and significant healthcare costs.
Methods
Introduced SPARK IT system for risk stratification and upskilling of pharmacists and nurses for non-inferior care.
Results
Achieved a 31% reduction in hypoglycemia prevalence and a 22% reduction in 30-day readmissions among diabetic inpatients from 2013 to 2021.
Conclusion
IDCP implementation improved diabetes care with sustained outcomes and expanded care capacity through staff upskilling and process optimization.
Lessons Learnt
Risk stratification with trained care teams improves clinical outcomes and cost-effectiveness.
