Aims
To achieve a timelier initiation of transitional feeding by creating a standardized guide for ward nurses.
Background
Transitional feeding often delays by 4-5 hours as ward nurses had to call dietitians for reviews, leading to inefficiencies in tube weaning.
Methods
Root cause analysis, PDCA cycle, and a feeding guide were piloted in three wards before being scaled hospital-wide. Results were monitored.
Results
Post-implementation survey (77 nurses) showed 88% used the guide; 93% reported time savings. Feeding tube weaning success increased from 75% to 88%, with no adverse events.
Conclusion
The guide effectively reduced delays in tube weaning and empowered nurses for timelier feeding transitions.
Lessons Learnt
Multidisciplinary teamwork was essential for success. Integrating the guide into NGEMR during planning ensures sustainability.
