Reduce unsuccessful referrals to dietitians for high LDL patients from 58% to 20%.
Background
Up to 58% of ASU patients with high LDL declined dietitian interventions, wasting resources.
Methods
Amended referral processes, created prompts in EPIC, revised education provided by neurology nurses.
Results
Decline rate reduced to 14% by March 2023; improved efficiency and patient education.
Conclusion
Enhanced referral process significantly improved patient care and resource utilization.
Lessons Learnt
Collaboration and stakeholder buy-in were critical for successful implementation.
