Heart failure admissions accounted for 17% of all cardiac hospitalizations, highlighting the need for better discharge planning.
Methods
Introduced personalized education, follow-up calls by patient navigators, and a Heart Failure Buddy mobile app.
Results
Reduced 30-day readmissions from 8.3% to 3.5% and 6-month readmissions from 13.6% to 5.8%; saved $116,400 annually.
Conclusion
Multidisciplinary care coordination significantly enhances patient outcomes and cost-efficiency.
Lessons Learnt
Personalized education and active follow-up are critical for reducing readmissions and improving care transitions.
