Methods
Redesigned workflow, teaching guidelines, and developed discharge folders with training slides.
Results
Caregiver confidence increased to 80%; 95% of complex care patients discharged within 1 hour. Bed savings of $10,000 from 200 saved bed days.
Conclusion
Enhanced discharge processes improve efficiency, reduce hospital stays, and increase patient and caregiver satisfaction.
Lessons Learnt
Structured workflows, checklists, and comprehensive teaching materials improve discharge outcomes.
