To increase the percentage of discharges before 11:30 a.m. from 13.38% to 30%.
Background
Inefficient discharge processes delayed patient transitions, impacting patient satisfaction and ward turnover.
Methods
Introduced discharge boards, TigerText reminders, and prepared discharge boxes for each nursing station.
Results
Discharge rate before 11:30 a.m. increased to 53.6% by December 2020, exceeding the target.
Conclusion
Structured discharge processes significantly improved patient flow and operational efficiency.
Lessons Learnt
Open communication and structured reminders motivate team members to adhere to improved workflows.
