Aims
To augment SGH care capacity by supporting early discharge and admission avoidance for patients whose treatment can be safely managed at home.
Background
Early discharge and home care reduce risks of hospital-acquired infections and improve patient comfort.
Methods
Conducted patient surveys, leveraged remote clinical monitoring, and established care protocols for SGH@Home program.
Results
89.7% of patients reported the experience exceeded expectations; 82.1% showed maintenance/improvement in EQ-5D scores; positive feedback emphasized comfort, safety, and efficient care.
Conclusion
SGH@Home provides an effective alternative to inpatient care, improving patient satisfaction and reducing hospital strain.
Lessons Learnt
Home-based care models require robust protocols, continuous feedback, and effective remote monitoring.
