Aims
Extend the benefits of Person-Centered Care (PCC) to high-care needs residents with at least 40% improvement in resident well-being, 40% improvement in purposeful engagement, and 20% improvement in staff PCC practices.
Background
The Jade Circle Model of Care (JC MoC) was launched in 2020 with support from Lien Foundation and Khoo Chwee Neo Foundation. It focuses on autonomy, choices, and dignity through PCC. The pilot at Jade Circle led to 61% improvement in well-being and 85% functional improvements.
Methods
Root cause analysis using the fishbone diagram identified staff competency gaps, lack of meaningful engagement, and task-focused care as barriers. Solutions focused on training staff, enhancing client engagement, and restructuring care delivery.
Results
Positive staff interactions improved by 50%, well-being scores increased by 61%, and client engagement rose by 41%, with a 30% boost in engagement time.
Conclusion
Sustainability efforts include fostering autonomy among staff, encouraging client decision-making, and conducting regular PCC training workshops and mentorship programs.

Tailored, person-centered care improves well-being and quality of life. Collaboration among staff, volunteers, clients, and community partners is key to sustaining PCC practices.
