Aims
To better manage frail patients during hospitalization by using a specialized Acute Frailty Unit (AFU).
Background
The increasing prevalence of frailty in Singapore’s aging population created a need for better management of frail patients.
Methods
Implemented a three-pronged approach consisting of team huddles, a frailty-ready environment, and person-centered care.
Results
Reduced the use of physical restraints, improved staff knowledge, and positively impacted patient care, especially for cognitively frail patients.
Conclusion
The AFU model improved patient outcomes and staff competence, and its practices are being extended to other wards.
Lessons Learnt
Collaboration across multidisciplinary teams and environmental design were key to the success of the AFU model.
