Results
Achieved improved patient compliance, reduced defaulter rates, and increased therapeutic outcomes through a holistic care approach.
Conclusion
The CDE program demonstrated the effectiveness of transdisciplinary care in improving patient outcomes and is now expanding.
Lessons Learnt
Communication and clear role definitions were essential in ensuring smooth integration of new roles.
