Aims
Develop a cluster-wide program to encourage speaking up for patient safety.
Background
A 2017 survey indicated that 57% of staff felt hesitant to ask questions when something didn’t feel right, surpassing the AHRQ's 35% threshold.
Methods
Adapted TeamSTEPPS® curriculum into TeamSPEAK®, incorporating critical language (CUS) and scenario-based videos. Piloted and refined through iterative PDSA cycles.
Results
Increased participant agreement on speaking up effectiveness during workshops; expanded rollout to SGH and other institutions.
Conclusion
TeamSPEAK® established a framework for open communication, reducing potential harm through better team collaboration.
Lessons Learnt
Structured training and leadership endorsement are critical for fostering a speaking-up culture.
