Aims
Reduce the turnaround time (TAT) for nursing home placement initiation by optimizing processes.
Background
Average TAT for referrals was 22.2 days in 2019 due to fragmented workflows, unclear criteria, and lack of holistic guidance.
Methods
Developed a Care Planning Package, streamlined workflows, conducted focus groups, and employed PDSA cycles.
Results
Reduced TAT to 14.8 days; avoided 2,109 bed days through improved efficiency and faster patient transitions.
Conclusion
The pilot demonstrated that optimizing workflows and providing resources significantly reduce delays and improve patient care.
Lessons Learnt
Standardized resources and proactive communication streamline NH placement processes effectively.
