Simplified referral form and process within the Agency for Integrated Care (AIC) system.
Results
Reduced median waiting time for transfers from 11 days to 5 days; referral submission time reduced from 3 to 1 day.
Conclusion
Streamlined AIC system referral processes for better patient outcomes and resource efficiency.
Lessons Learnt
Simplifying referral processes can drastically reduce waiting times and improve bed utilization.
