Aims
Improve Berg’s Balance Score (BBS) by at least 20%, prevent deterioration of Modified Barthel Index (MBI) by more than 5%, and achieve at least 90% satisfaction and motivation after 16 sessions.
Background
Kwong Wai Shiu Care @ St George offers a Community Rehabilitation Programme for outpatient seniors, with traditional models leading to dropouts after step-down to Maintenance Rehabilitation.
Methods
Deming PDCA methodology was used. PLAN phase included identifying suitable seniors, planning program logistics, and designing engaging exercises. DO phase implemented the program with continuous rehabilitation assessment using MBI and BBS.
Results
All clients demonstrated progress in line with objectives. Client 1 improved BBS by 30.9% and MBI by 12.4%. Client 2 maintained MBI within the 5% limit. Client 3 improved BBS by 60% and MBI by 13.9%. 100% satisfaction and motivation were reported.
Conclusion
A new protocol was developed for everyday workflow. Future plans include recruiting more seniors, expanding program duration, and involving Occupational Therapy activities.
Lessons Learnt
Shared goals and social engagement helped seniors sustain participation, making rehabilitation enjoyable and motivating.
