Aims
The aim of the SPACE@JCH program is to provide a home-like environment in the wards for patients to practice skills repetitively and empower caregivers with skills, knowledge, and space to facilitate skill acquisition with patients outside of formal rehabilitation sessions.
Background
The program was developed to address the need for a home-like environment in the ward to facilitate patient and caregiver learning and provide digital infrastructure for tele-consultation for care follow-up post-discharge. It is innovative as it prepares patients to transition back to their home using a simulated home-like environment for rehab and caregiver training.
Methods
The program includes a home-like environment in the ward, periodic exposure to a 3-room HDB apartment mock-up rehabilitative space for caregiver training, and a Mobility Park for community integration training.
Results
The program exceeded its targets with 81.5% of rehab patients achieving MBI10, a 29% increase in caregiver training, and 83% of rehab patients undergoing post-discharge tele-consult sessions.
Conclusion
The SPACE@JCH program is effective in providing a home-like environment for rehabilitation and caregiver training, and it is being scaled to six other wards within Jurong Community Hospital.
Lessons Learnt
A mindset shift is needed for all care team members, including patients and caregivers, to recognize that rehabilitation can occur beyond direct contact hours. Tele-consultation has limitations and needs to be complemented with in-person sessions. Digital literacy enhancement is important for successful tele-consultation.
