Ensure stable medication source before PTPNH operational.
Background
Existing medication suppliers were unable to meet the expanded needs of Kwong Wai Shiu Hospital for Potong Pasir Nursing Home.
Methods
Collaborated with NHGP and other stakeholders to partner with Kallang Polyclinic and TTSH. Developed workflows to address formulary gaps and improve supply chain.
Results
Established stable medication source via partnerships, ensuring continuity of care and cost management.
Conclusion
Stable medication supply chain allows effective care delivery for nursing home residents while optimizing costs and reducing medication waste.
Lessons Learnt
Self-sufficiency and innovative approaches like colored prescription papers for location differentiation improve system resilience.
