Aims
To increase the activation of residents for screening for life (SFL) with GP clinics and to anchor preventive care.
Background
Address the gap of unscreened eligible residents for SFL and promote GP-patient preventive care relationships.
Methods
Engaged GPs, introduced lifestyle interventions via Community Health Teams (CHT), and facilitated easy sign-ups for SFL.
Results
Achieved 43% attendance rate among signed-up residents and enhanced follow-ups. Established ongoing relationships between residents and GPs.
Conclusion
Enhanced preventive care and stronger GP-patient bonds demonstrate potential for scalable impact.
Lessons Learnt
Limited GP clinic options posed challenges; automation and expanded engagement recommended.
