Challenges in sepsis management include difficulty establishing reliable baseline data, manual data collection burden, variable buy-in from different specialties and departments, resource constraints, and the need for continuous education due to staff turnover.
Methods
PDSA cycles including creating a Sepsis Care Bundle Workflow, sharing initiatives with healthcare teams, educational sessions for nurses, and data coaching sessions with nurse champions.
Results
Improvement from a pre-intervention median time of 220 mins to a post-intervention median time of 159 mins for antibiotic administration. The percentage of febrile patients meeting the =180 mins target increased from 37% pre-intervention to 62% post-intervention. Reduction in median length of stay from 10 days to 9 days, saving 1 bed day per patient, translating to monetary savings.
Conclusion
The intervention shows promise in improving the timeliness of antibiotic administration in sepsis cases, leading to better patient outcomes and cost savings.
Lessons Learnt
Importance of integrating sepsis screening into daily practice, ongoing training and engagement, leadership and accountability, real-time monitoring, encouraging multidisciplinary collaboration, and continuous improvement through audits and feedback.
