Aims
Double the percentage of ASU patients on texture-modified (TM) diets referred for DT management from 22% to 44% by June 2020.
Background
Studies show stroke patients on TM diets are 2.6x more likely to suffer malnutrition, leading to increased mortality and longer hospital stays.
Methods
Conducted awareness talks, changed workflows to allow Speech Therapists (STs) to refer patients directly, and implemented monthly reminders for referrals.
Results
Referrals increased from 22% to 100% by June 2020; sustained improvement after consistent reminders.
Conclusion
Early identification and intervention successfully reduced malnutrition risk among stroke patients.
Lessons Learnt
Stakeholder buy-in, workflow changes, and regular reminders were key to achieving sustainable improvements.
