Conducted value stream mapping, root cause analysis, and process optimization to improve scheduling and communication.
Results
Reduced lead time from 23 to 7 working days, surpassing the initial goal. Clients now receive timely rehabilitation within a week post-discharge.
Conclusion
The project optimized referral processes, improved patient outcomes, and set a new benchmark for home rehabilitation lead times.
Lessons Learnt
Standardizing follow-ups and improving scheduling efficiency are critical for reducing wait times.
