Aims
Reduce the proportion of patients on I/O Chart Monitoring in an Intermediate Rehabilitation and Subacute Setting.
Background
Inpatient charting for intake and output (I/O) takes considerable nursing hours and resources, particularly for medically stable patients.
Methods
Modified the Nurse-led I/O Chart Review Process, empowering junior nurses and healthcare assistants to review and discontinue unnecessary I/O monitoring.
Results
Reduced the proportion of patients on I/O chart monitoring from 73% to 43.6%, saving an average of 48 minutes of nursing time per day.
Conclusion
The new process improved efficiency, reduced unnecessary charting, and allowed nurses to focus on more critical care tasks, benefiting patient rehabilitation.
Lessons Learnt
Empowering junior staff to lead the process helped to integrate the change into daily practice. Providing feedback and continual support ensured sustainability.
