Aims
To reduce MERP Category D Insulin Medication Errors by 50% over 3 years at Khoo Teck Puat Hospital (KTPH).
Background
An uptrend in insulin-related medication errors was noted in 2020-2021, with significant risks to patients, prompting the initiative to improve medication and patient safety.
Methods
Interventions included the initiation of a Hyperkalaemia Kit, improving the interface for displaying blood glucose monitoring records, and setting up an Admin-Too-Close best practice advisory prompt.
Results
The longest number of days between events improved from 82 days in 2022 to 135 days in 2023. Total number of MERP D and above insulin-related medication errors decreased from 7 in 2022 to 3 as of August 2024.
Conclusion
The project shows promise in reducing insulin-related medication errors, with ongoing initiatives to further improve outcomes.
Lessons Learnt
Multidisciplinary collaboration is crucial to avoid medication errors and improve patient care, accuracy of prescriptions, and clinical decision-making.
Additional Information
Zero MERP F to I insulin-related medication errors from 2023 to August 2024. Reducing error rates prevents medication-related mortality and morbidity.
