Background
Fall incidence was high compared to other hospitals; patients aged ≥65 with multiple comorbidities.
Methods
Kaizen “Go See, Ask” approach; interventions introduced using PDCA cycles; fall sensor mats, acuity-based staffing, RCAs.
Results
Inpatient falls reduced by 0.4 per 1,000 patient days; improved teamwork and patient safety.
Conclusion
Transformational changes in care and teamwork led to safer environments with reduced fall rates.
Lessons Learnt
Sustained monitoring and leadership support are key for continuous fall prevention improvements.
