Aims
Achieve zero broken skin incidents for 6 months and reduce patient discomfort and additional treatment.
Background
Between 2014-2017, SNEC reported 37 broken skin incidents. Patients, primarily elderly, had fragile skin due to age or medication therapy.
Methods
Adopted PDSA cycle; developed new skin assessment checklists; conducted education sessions.
Results
No broken skin incidents among high-risk patients identified with revised assessment criteria in the final phase.
Conclusion
Collaborative assessment processes can be cost-effective and reduce patient harm with minimal workflow disruption.
Lessons Learnt
Simplified inclusion criteria improve identification accuracy and workflow efficiency.
