Background
Pressure injury rates increased to 0.09 per 1,000 patient days in 2020, exceeding KPIs.
Methods
Solutions included thorough skin assessments, 2-hourly positioning schedules, staff education, and pressure point photo capture.
Results
Pressure injury rates reduced to 0.04 per 1,000 patient days; zero cases reported in Jan-Feb 2021.
Conclusion
Sustained preventive care through audits and visual reminders successfully reduced injuries.
Lessons Learnt
Early detection through skin assessments and timely care were critical; staff training boosted compliance.
