Aims

Eliminate the incidence of home medication errors for patients admitted to Ward B5 by October 2018.

Background

Between Q1 2017 and Q2 2018, 14 medication errors were recorded, with 6 involving patients consuming home medications without nurse knowledge.

Methods

Implemented photo-stands as visual reminders, family education sessions, staff engagement, double-checking systems, and regular quality reviews.

Results

Achieved zero medication errors for 2 consecutive quarters. Nurses’ compliance remained at 100%.

Conclusion

Sustained results with regular reinforcement, ensuring compliance becomes standard practice for medication safety.

Lessons Learnt

Visual cues and simple tools such as photo-stands, combined with family education and staff accountability, drove success.
