Methods
Root cause analysis; implemented workflows for means testing, home modifications, and caregiver training.
Results
Reduced LOS to 27.4 days; time for home photos reduced from 8.3 to 6.5 days.
Conclusion
Workflow redesign effectively reduced LOS; scalable across rehabilitation units.
Lessons Learnt
Standardized processes improve discharge efficiency and reduce delays.
