Implemented A-G Assessment tool and SBAR communication framework through structured training, simulations, and e-learning.
Results
Code Blue activations reduced by 40%. Nurses' confidence in identifying and escalating deteriorating patients improved significantly.
Conclusion
A-G Assessment and SBAR successfully standardized escalation processes and improved outcomes.
Lessons Learnt
Structured tools and training empower nurses to act promptly, improving patient safety outcomes.
