Aims
To streamline the treatment process for patients with malfunctioning indwelling dialysis catheters and reduce the time to discharge
Background
Indwelling dialysis catheters often malfunction, leading to prolonged hospitalizations. This project aimed to reduce the time to discharge by streamlining processes for the treatment of poor-functioning catheters.
Methods
Implemented a direct referral process, virtual wards, and revised protocols for treating catheter dysfunction, reducing unnecessary ED visits and hospital stays.
Results
Successfully reduced ED visits and admissions by 58%, reduced readmission rates, and decreased hospital stays for patients with dialysis catheter issues.
Conclusion
The implementation of streamlined protocols significantly reduced hospital stays and improved patient care, leading to significant cost savings.
Lessons Learnt
Improved processes through faster diagnosis, better communication, and reduction of time in ED. Implementing a virtual ward system resulted in more efficient patient management.
