Aims
Increase and sustain NCS utilization for chronic disease patients at Outram Polyclinic to 25%.
Background
NCS utilization rates were consistently below 15%, exacerbated during the COVID-19 circuit breaker period.
Methods
Family physicians reviewed eligible patients, time-matched slots, and NCS nurses conducted care sessions with adjacent doctor support.
Results
Sustained NCS utilization rate above 40%; 72% patient awareness, 64% rated NCS as “Excellent.”
Conclusion
NCS successfully enhanced care delivery for stable chronic disease patients, improving efficiency and outcomes.
Lessons Learnt
Inter-professional collaboration and nurse-doctor support improved right-siting and patient care continuity.
