Aims

Care for seniors was fragmented, requiring collaborative partnerships for improved care.
Methods
Developed and applied a survey based on GRPI and Collaboration Advantage Framework.
Results
Goal alignment and role clarity improved; CoCs progressed to higher levels of collaboration.
Conclusion
CoC partnerships strengthened collaboration and demonstrated the value of intentional network building.
Lessons Learnt
Structured tools can measure intangible benefits of partnerships; clarity in roles improves outcomes.
