Aims
To increase referral rates to Weight Management Clinic (WMC) among obese patients in Ang Mo Kio Polyclinic from 0% to 9% in six months.
Background
Low baseline referrals to WMC among obese patients despite high obesity prevalence in Singapore.
Methods
Patient education, motivational interviewing, and multimedia awareness
Results
Referral rates increased to 1.73%. Obesity counseling rates improved from 13.2% to 52.3%.
Conclusion
Integrated strategies enhanced referrals and patient engagement but fell short of targets.
Lessons Learnt
Multimedia and staff engagement improved awareness and outcomes but faced system and language barriers.
