Admission rates reduced to 40.2%, exceeding the target; 0% re-attendance rate achieved consistently for intervened patients.
Conclusion
Frontloaded PT interventions in ED successfully reduced hospital admissions and prevented re-attendance, enhancing care quality.
Lessons Learnt
Collaborative multidisciplinary approaches and patient empowerment are essential for sustainable improvements.
