Aims
Implement and sustain telemedicine during and beyond COVID-19, using a hybrid home care model.
Background
Telemedicine was essential during the pandemic to provide safe, quality care for palliative home care patients.
Methods
Implemented telemedicine via video consultations (VC), focusing on stable patients initially. Expanded to include unstable phases post-success.
Results
804 VCs conducted; VC adoption increased 4.8-fold from 2020 to 2022. 82% of patients/families reported being "satisfied" or "very satisfied".
Conclusion
Telemedicine enhanced the care model, balancing technology and human touch to improve patient experience.
Lessons Learnt
Dedication to embracing telemedicine improved patient care efficiency, especially during after-office hours.
