Aims
To review and improve on step-down care and discharge processes to enhance patient flow and overall patient experience.

Methods
Multidisciplinary teams evaluated step-down care and discharge procedures, revising caremodels to ensure right-siting of patients. Interventions included assigning patients into care streams and driving ambulatory and elective surgery.
Results
From May to October 2024, clinicians flagged 57 patients to Inpatient Services to expedite discharge, achieving a median discharge time of 6 days. The average weekly fit long-stayers dropped from 23 in May 2024 to 7 in October 2024.
Conclusion
The multidisciplinary initiatives improved overall hospital experience for patients and fostered a culture of continuous improvement.
Lessons Learnt
The collaborative efforts of different departments enhanced patient flow and ensured timely discharges, exemplifying the power of a unified vision.
