Aims
To assess the feasibility and effectiveness of a redesigned hypertension care model leveraging technology for self-monitoring and tele-consultations.
Background
Increasing patient load and comorbidities required new delivery models to manage chronic diseases effectively while reducing clinic visits.
Methods
Pilot study using tele-monitoring, 4G gateway blood pressure devices, chatbots, and remote care manager interventions. Compared 103 PTEC patients vs. 115 usual care patients.
Results
Greater reduction in systolic blood pressure for PTEC patients. Reduced face-to-face visits (1.1 vs 1.5) and improved patient convenience. Man-hour savings equivalent to 5 FTEs/year. Cost-effective at $16,873/QALY.
Conclusion
PTEC effectively improved hypertension care, reduced workload, and proved cost-effective, demonstrating feasibility for large-scale deployment.
Lessons Learnt
A multi-disciplinary team and leadership engagement were critical. Tele-monitoring showed quick wins and relevance during COVID.
Additional Information
PTEC was critical during COVID-19 as medications were titrated remotely.
