Aims
To reduce CLABSI rates in neurosurgical ICU patients from a median of 6.9 per 1,000 central line days to 0 per 1,000 central line days in 1 year.
Background
CLABSI is associated with significant morbidity, mortality, and financial burden. TTSH ICUs reported the highest CLABSI cases in 2017 compared to restructured hospitals.
Methods
Interventions included education on chlorhexidine drying time, changing OT drapes, and adding CVL reviews to ICU daily ward rounds. Conducted PDSA cycles and monitored adherence.
Results
CLABSI rates reduced from 6.9 to 3.95 per 1,000 central line days. ICU bed day savings: 8.2 days per infection prevented. Cost savings: $17,056 per prevented infection and $1,699.39 per patient.
Conclusion
Interventions successfully reduced CLABSI, resulting in improved patient safety, reduced morbidity, and substantial cost savings.
Lessons Learnt
Buy-in from all stakeholders, root cause analysis using flowcharts and Pareto diagrams, and sustained interventions ensured success.
