Aims
To preserve memories, facilitate legacy building, and strengthen bonds among residents and families
Background
Chronic sick residents faced challenges due to sudden health changes, limited autonomy, and communication barriers. Families struggled with anticipatory grief.
Methods
The program involved family photo-taking, reminiscence sessions, and the use of strength perspective to empower families to reflect on positive memories and bond with residents
Results
89 families and 26 residents engaged; feedback showed a meaningful impact on families and residents, creating a sense of closure and connection
Conclusion
Enhanced sense of community and active participation through sensory therapeutic gardening
Lessons Learnt
Structured reminiscence and family bonding sessions improved emotional well-being of families. Future expansion of garden is planned
