To facilitate conversations about future healthcare needs based on personal values, goals, and preferences.
Background
Addressing healthcare preferences early provides clarity and reduces stress during health crises.
Methods
Four-session program using psychoeducation, experiential activities, and group sharing, including ACP and LPA discussions.
Results
83% of participants completed an ACP. Improved awareness of personal values and confidence in discussing healthcare needs.
Conclusion
The program fosters preparedness and confidence among seniors, supporting better healthcare planning.
Lessons Learnt
Partnerships with community organizations enhance program outcomes and reach.
