Aims
To establish a population-based medication management system to resolve inappropriate polypharmacy.
Background
Polypharmacy in elderly patients led to adverse drug-related problems and increased hospital admissions.
Methods
Implemented a multidisciplinary approach with medication reviews, assessments, and collaborative care involving family physicians and pharmacists.
Results
Reduced unplanned admissions by 23.8% and improved drug-related problems (DRP) resolution rate to 80.7%.
Conclusion
The model demonstrated that early medication reviews can improve clinical outcomes by reducing hospital admissions and improving the management of chronic conditions.
Lessons Learnt
Engaging patients and caregivers, and involving all stakeholders early in the process were critical for success.
