Aims
The Revitalize programme aims to improve community health and social care by providing personalised exercise strategies tailored to each senior's frailty status, encouraging long-term participation, functional improvement, and community well-being.
Background
Frailty among older adults in Singapore is projected to rise significantly, and recognising frailty as a dynamic and reversible condition, the Revitalize programme offers early intervention through a neighbourhood-based model delivered at Ren Ci Woodlands.
Methods
The programme involves personalised, twice-weekly sessions over six months, with assessments at pre-, mid-, and post-programme stages using the Short Physical Performance Battery (SPPB), fast gait speed (10-Metre Walk Test), and handgrip strength. Therapy Assistants deliver interventions, and family involvement is encouraged.
Results
Mid-point assessments show promising results, with 42.9% demonstrating significant gains in fast gait speed and 85.7% maintaining or improving their SPPB scores. Handgrip strength showed positive gains, with 85.7% maintaining or improving in the left hand and 100% in the right.
Conclusion
Revitalize is ready to be extended to other Ren Ci DRCs, benefitting more seniors in the community. The programme contributes to building an effective manpower-optimised model that empowers seniors to stay active, independent, and socially connected.
Lessons Learnt
Feedback from participants and family members is being incorporated to ensure interventions remain effective and sustainable. The programme's approach to involving family members and fostering peer support supports social engagement and sustainability.
