To progressively roll out the Be Fit, not Frail! (BFF) programme to Active Ageing Centre partners and to facilitate early detection and management of frailty.
Background
NHG's AgeWell SG Frailty Programme is aligned with the MOH's National Frailty Strategy Policy and the WHO's ICOPE framework.
Methods
Residents undergo screening for intrinsic capacity losses followed by personalized care plans and appropriate referrals.
Results
Through early detection and interventions, frailty can be prevented, reversed, or managed, even in its advanced stages.
Conclusion
The BFF programme is a comprehensive community frailty programme that supports healthy living and ageing.
