Aims
To integrate comprehensive care services for seniors and persons with intellectual disabilities (PwID) to enable them to live and age in their own community.
Background
The St Hilda’s Community Services Centre (SHCSC) model of care integrates various services to provide 360 care for seniors. The enhanced model aims to include the underserved ID health sector.
Methods
SHCSC collaborates with various stakeholders, including government agencies, healthcare providers, and non-profit organizations, to provide comprehensive services for PwIDs and families.
Results
SHCSC has successfully outreached to the Jalan Batu estate, enabling residents to participate in community programs and services. Mr. L, a PwID, receives active rehab and other support services.
Conclusion
The enhanced care model shows promise in providing comprehensive community-based options for PwIDs, fostering an inclusive community.
Lessons Learnt
Strengthening support structures via technological innovation and collaboration with the wider community is vital for deeper integration of PwIDs within society.
