Aims
To improve the percentage of low back pain patients in ED achieving a PSFS walking score of ≥7/10 one week after discharge from 25% to 75% within 6 months.
Background
Preliminary data showed only 25% of patients in ED achieving PSFS walking score ≥7/10 after discharge. The project identified care gaps and aimed to address them using a multi-disciplinary approach.
Methods
Utilized CPIP methodology, root cause analysis, PDSA cycles, and multi-disciplinary collaboration with ED doctors and nurses.
Results
Significant improvement in PSFS scores from baseline 25% to a range of 66%-100%. Positive feedback from patients and staff highlighted better patient management and clinical knowledge sharing.
Conclusion
Multi-disciplinary collaboration improves clinical outcomes for low back pain patients in ED, fostering teamwork and better management practices.
Lessons Learnt
Acute pain patients require supportive care and education to overcome psychosocial factors and generate favorable outcomes. Continuous learning and teamwork are critical for success.
