Results
PROMs improved significantly for Quality of Life and healthcare experience. Functional status improvements were mixed.
Conclusion
The GSH is a viable model for managing community frailty through tailored care plans and multidisciplinary coordination.
Lessons Learnt
Patient motivation and tailored education sustain behavioral changes and PROM improvements.
